Joseph P. Addabbo Family Health Center, Inc. is a Federally Qualified Health Center, Level 3 Patient-Centered Medical Home, a model of safe and quality primary care that is affordable and accessible. Our patients are treated with respect, dignity, empathy and compassion, which encourages the development of strong, trusting relationships between patients and their providers.

History

Established as a free pediatric clinic in 1981, Joseph P. Addabbo Family Health Center, Inc. was incorporated in 1987 and named for Rep. Joseph P. Addabbo, Sr., who championed the cause of affordable healthcare.

For more than 20 years, Congressman Addabbo passionately represented the Rockaways and Southeast Queens area in Congress. We have continued his legacy. For more than 30 years, Joseph P. Addabbo Family Health Center, Inc., has demonstrated its ability to overcome barriers and improve access to healthcare for individuals and families in our community. We are an ever-expanding Federally Qualified Health Center (FQHC) with five operational health centers located on the Rockaway Peninsula, in Southeast Queens, and Red Hook, Brooklyn. Addabbo has been designated as the highest-level (Level III) Patient-Centered Medical Home where our caring professional staff of physicians, nurses and support staff offer a wide range of services from routine appointments to “walk-in” service for sick visits. As a Legacy FQHC (30 years of providing exemplary healthcare), we have a long history of delivering a full range of high-quality, accessible and cost-effective primary, preventive, specialty and enabling services to all those who walk through our doors.

We treat patients equally without regard to socioeconomic status, age, race, color, ethnicity, national origin, religion, culture, language, physical or mental disability, sex, sexual orientation and gender identity or expression. Joseph P. Addabbo Family Health Center, Inc. patients are not denied services due to inability to pay. A sliding scale discount program is available.

Our mission is to be the leading preventive and comprehensive primary healthcare provider in New York. We are committed to providing services that are patient-focused, high-quality, accessible and cost-effective. Services will be provided with respect to the social, religious, cultural and economic needs of the communities we serve. All patients are serviced regardless of ability to pay. Through education, innovation and community partnerships, we will anticipate, and exceed, the expectations of our patients and communities.

Building a Healthy Community

One of the keys to building a healthy community is to understand the issues that affect it the most. Because the health of the community is part of our Mission Statement, YOUR concerns are OUR concerns. The members of our Board of Directors also help us define those needs and concerns. We participate in health fairs and community meetings, provide speakers for local organizations and schools as well as host educational programs.

Joseph P. Addabbo Family Health Center, Inc. is a private, freestanding, nonprofit 501(c)(3) Diagnostic and Treatment Center (DT&C) authorized to deliver health and medical services pursuant to Article 28 of the New York State Public Health Law.

FTCA Deemed Organization. The Joseph P. Addabbo Family Health Center is granted medical malpractice protection through the Federal Tort Claims Act (FTCA). The Joseph P. Addabbo Family Health Center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
