Sliding-Fee Discount Program

The mission of The Joseph P. Addabbo Family Health Center is to ensure that all patients and community members have 100% access to the quality health care they need. To help make this possible, we have created a Sliding-Fee Discount Program, which allows us to adjust visit costs depending on the patient’s eligibility for the program. The Sliding-Fee Discount Program is based on an individual’s income and family size. No one is denied care because of an inability to pay.

In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, the USDA, its Agencies, offices, and employees, and institutions participating in or administering USA programs are prohibited from discriminating based on race, color, national origin, sex, disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.

Overview

The Sliding Fee Discount Program offers reduced rates for eligible patients on a variety of services, including copayments, coinsurance, deductibles, and certain services not covered by insurance. Discounts are determined using the current Federal Poverty Guidelines, ensuring that patients with lower incomes pay less for their care.

Eligibility Criteria

All patients are encouraged to apply, regardless of insurance status. Eligibility is based solely on household income and family size. Patients who are uninsured, underinsured, or have insurance with high out-of-pocket costs may qualify for discounted services.

Application Process

Patients may request an application at any of our health center locations or contact the billing office to obtain the necessary forms. To complete the application, documentation verifying household income is required, such as recent pay stubs, tax returns, or benefit statements. To maintain enrollment in the program, patients must reapply annually or whenever their financial circumstances change.

If You Do Not Qualify

Patients who do not meet eligibility criteria for the Sliding Fee Discount Program but are unable to pay their medical bills in full may arrange a payment plan. Our staff is available to assist in establishing manageable payment arrangements to ensure continued access to care.

Commitment to Care

No patient will be denied services due to an inability to pay. If you have concerns about the cost of your care, please speak with a case manager or financial counselor. We are dedicated to working with all patients to identify solutions that promote health and financial well-being.
