At The Joseph P. Addabbo Family Health Center (JPAFHC), we believe healthcare should be accessible, compassionate, and centered around you. For nearly 40 years, we have proudly served the communities of Southeast Queens and Red Hook, Brooklyn, providing high-quality, community-based healthcare for generations of families. As a Federally Qualified Health Center (FQHC), we are committed to expanding access to coordinated, patient-centered care regardless of a patient’s ability to pay.
At JPAFHC, we deliver integrated, whole-person care that combines medical, behavioral, dental, and supportive services under one healthcare network. Our care model focuses on prevention, early intervention, chronic disease management, and wellness while building strong relationships with the communities we serve.
We are committed to providing culturally responsive care rooted in compassion, dignity, equity, and respect.
We provide a wide range of primary and preventive care services at JPAFHC, including routine checkups and physicals, as well as specialized care such as chronic disease management. We also provide women's health and family planning services, pediatric care, and dental and oral health services for the whole family. Patients also have access to behavioral health, podiatry, and pharmacy services at JPAFHC, and telehealth appointments are available to help make patient care more accessible.

Our History
Founded in 1981 as a free pediatric health center, The Joseph P. Addabbo Family Health Center, Inc. was officially incorporated in 1987, honoring Rep. Joseph P. Addabbo, Sr., a tireless advocate for affordable healthcare. For over 30 years, we have continued his legacy, overcoming barriers and improving healthcare access for individuals and families in our community. We now operate five health centers across the Rockaway Peninsula, Southeast Queens, and Red Hook, Brooklyn, providing a full range of high-quality, accessible, and cost-effective healthcare services.

Board of Directors
The Joseph P. Addabbo Family Health Center, Inc., a nonprofit Federally Qualified Health Center, is governed by a volunteer Board of Directors made up of a diverse and representative group of patients and community leaders who donate their time and talent to further the mission of the Health Center.
