Queens, NY –  The Joseph P. Addabbo Family Health Center, Inc. (JPAFHC) is making remarkable strides in combating diabetes and improving chronic disease management with its innovative Diabetes Management Initiative. Since launching 18 months ago, this program has successfully improved health outcomes for over 500 high-risk patients by integrating cutting-edge technology and comprehensive patient education. 

The Diabetes Management Initiative at JPAFHC uniquely incorporates the use of the Dexcom G7 Continuous Glucose Monitoring (CGM) System, which allows for real-time blood sugar tracking. Providers in the program utilize robust patient education on chronic disease management to empower individuals to manage their health effectively.

The High-Risk Diabetes Practice at JPAFHC was established to address specific needs of patients with an HbA1c of 8.5 or greater. Individuals living with diabetes are at a heightened risk for cardiovascular events, including heart attacks and strokes, especially when compounded by conditions such as hypertension and hypercholesterolemia. By aggressively managing hyperglycemia, the program helps patients mitigate these risks and improve overall health. 

Dr. Kecia Ford, MD, Internal Medicine, highlighted the importance of comprehensive diabetes care: “Individuals living with diabetes are twice as likely to experience heart attacks, strokes, and heart failure compared to those without diabetes. Each year, the American Heart Association’s Go Red for Women campaign highlights the importance of managing blood sugar levels to reduce cardiac risks. Our High-Risk Diabetes Clinic utilizes education and technology to empower patients, helping them gain control over their diabetes, heart health, and overall well-being. We are excited to offer this service to our Addabbo community.”

Patients who have participated in the program have experienced life-changing improvements. In addition to helping patients achieve better blood sugar control, the initiative underscores the importance of regular medical follow-ups and open communication with one’s provider. Dr. Ifeanyi Oguagha, MD, Internal Medicine, emphasized the long-term benefits: “Consistent control of blood sugar levels helps slow and prevent complications from diabetes, such as damage to the eyes, nerves, and kidneys. Regular medical follow-ups ensure monitoring, adjustments in treatment, and early intervention, leading to better overall health outcomes.”
