The Joseph P. Addabbo Family Health Center (JPAFHC) proudly announced that it has received Patient-Centered Medical Home (PCMH) Recognition from the National Committee for Quality Assurance (NCQA), a nationally recognized organization dedicated to improving the quality of healthcare.
The Patient-Centered Medical Home designation recognizes healthcare organizations that demonstrate excellence in delivering coordinated, accessible, and patient-centered care designed to improve health outcomes, strengthen patient engagement, and enhance the overall patient experience.
This recognition underscores JPAFHC’s continued commitment to integrated, whole-person healthcare and advancing health equity across Southeast Queens and Red Hook, Brooklyn.
