HIV Prevention and Care Services

Here at JPA, we aim to deliver high quality, culturally affirming health care and services at every point of care to support optimal health for people with or without HIV. We work to empower patients with knowledge of their status and the tools to manage their health.

HIV Prevention Services

We help HIV negative patients stay negative with the provision of:

  • FREE and confidential rapid HIV testing at all our sites – Results in 20 minutes!
  • Access to condoms and sexual health education
  • PrEP and PEP
  • LGBTQ+ support groups
  • Case Management Support
  • Community-based outreach – testing, counseling and prevention education.

HIV Care and Treatment

We support patients from diagnosis to becoming HIV healthy through the provision of caring, confidential and holistic treatment. Our goal is to support patients living with HIV/AIDS towards achieving and maintaining their medical care as well as other psychosocial supports.

We help people living with HIV/AIDS achieve optimal health through the provision of:

Comprehensive Medical Care:

  • HIV medical care and primary care from providers who specialize in HIV care
  • Health education and nutrition counseling
  • Treatment education, including the importance of taking medication on a regular basis
  • Women’s health services, including gynecology, PAP tests, family planning and birth control
  • LGBT-sensitive services
  • Referrals for specialty care services
  • In-house pharmacy services.

Care Coordination and Case Management

  • Patient accompaniment to appointments and other assistance navigating care
  • Assistance with partner-notification
  • Transportation assistance
  • HASA referrals
  • ADAP applications (AIDS Drug Assistance Program)
  • Help with M11Q forms
  • Medicaid, Public Assistance SSI/Disability, etc., assistance
  • Substance use counseling and treatment referrals.

These services are supported by funding from various Federal and State entities including Human Resources and Services Administration (HRSA), NYS Department of Health, Substance Abuse and Mental Health, Gilead
