Joseph P. Addabbo Family Health Center is currently working towards becoming a Center of Excellence in Diabetes and Obesity and offers a comprehensive array of health services for infants, children, adults and seniors at a location near you.

Adult Medicine/Family Practice: Our entire team of physicians, nurses and staff is committed to providing you and your family with expert, compassionate, culturally sensitive patient care. As a recognized Level III (highest level) Patient-Centered Medical Home, our focus is on your individual healthcare needs. We can help you make informed decisions about your health needs and provide the tools and education you need to stay healthy. We are your “home base” for comprehensive healthcare.
Designed with your wellness in mind, we provide regular internal medicine physicals and chronic disease management for conditions such as diabetes, high blood pressure, vaccinations and screenings. We also provide podiatry, HIV counseling, nutrition, OB/GYN and other specialties. If your health warrants a referral to a specialist, we will monitor and coordinate your care as it moves forward.
Joseph P. Addabbo Family Health Center provides a full range of medical care for your entire family, from well-baby care through adolescence to adult medicine. We offer comprehensive primary care medical services at our five conveniently located medical centers with experienced physicians and caring staff. Our high standard of medical care is delivered via the Family Practice model.

Asthma & Allergy Care: We know that living with asthma and allergies can be challenging, especially when they result in side effects. Our physicians are trained in all areas of asthma and allergy treatments and can help you determine your trigger sources so that you can manage your condition more effectively.

Behavioral Health: All patients are evaluated by a mental health professional when they are referred to our Behavioral Health program by their primary physician. Our professional staff works as a team to complete a full assessment and develop a treatment plan suited to the needs of each patient. Our staff treats patients in a respectful and culturally sensitive manner. Families play an important role for our mental health patients; we educate them about the patient’s illness and individual treatment plan.

Dental Services: You deserve to have a beautiful smile! Our skilled professional dental team offers FREE dental screenings, gentle dental exams, cleanings, filling, extractions, root canals, minor oral surgery, crowns plus full and partial dentures. We now use the very latest, state-of-the-art technology, the Gendex Digital Orthopantomogram, which can take comfortable X-rays of the entire mouth and jaw area in less than 15 seconds. Our experienced dentists are experts at helping you relax as we help you achieve a healthy, happy smile!

Department of Wellness & Social Services: Our HIV Program provides care for HIV-positive patients. Our comprehensive HIV Specialty Care is based on the principles of accessibility, confidentiality, health promotion and patient satisfaction. It includes medical visits, lab work, PAP smears every six months, TB screenings, immunizations for hepatitis A and B, pneumonia vaccine as needed, annual flu vaccines and referrals for preventive care. We provide HIV counseling and testing as well as referrals for PrEP and PEP.

Endocrinology: Our endocrinologists are specially trained physicians who diagnose diseases related to your glands. Typically, we treat patients who suffer from hormonal imbalances, menopause, diabetes, metabolic disorders, lack of growth, osteoporosis, thyroid diseases, cancers of the endocrine glands, over- or under-production of hormones, cholesterol disorders, hypertension and infertility.

Health Insurance Access: Our Insurance Enroller, IPA/Navigator program provides health insurance enrollment services to potential and current enrollees. Our staff educates potential and current enrollees about the New York State of Health – the Official Health Plan Marketplace and the numerous health insurance plans available – and provides education on and facilitates enrollment into Medicaid, Child Health Plus, Essential Plan and Qualified Health Plans.

Our culturally and linguistically diverse IPA/Navigators are responsible for managing the various application stages from inception to retention. We assist potential and current enrollees with appeals, grievances, complaints and/or questions regarding their health insurance. Additionally, HIAP staff provides help to small businesses that are providing insurance to their employees through the Small Business Health Options Program (SHOP). Our In-Person Assistors (IPA) are available to help you at four out of our five locations (Site 1, Site 2, Site 3 and Site 6). Additionally, we have an off-site representative at Achiezer, a community resource center for the Jewish population.

Infectious Disease: Infectious diseases are commonly in the news because they kill more people worldwide than any other single cause. Infectious diseases that typically affect our patients are common cold, influenza (flu), hepatitis, pneumonia, sexually transmitted infections (STIs), and strep.

Laboratory: For our patients’ convennience, Joseph P. Addabbo Family Health Center offers on-site lab services, bringing our patients another step closer to accessing better healthcare and early diagnoses. We offer FREE pregnancy testing, rapid FREE HIV testing for the uninsured, WIC Program eligibility testing, complete blood work-up, diabetes testing and more.

Nutrition: Your medical physician may recommend that you consult with a nutritionist to promote your good health. A balanced, nutritious diet is at the foundation of a healthy lifestyle. Many diseases can be prevented simply by following good nutritional habits. Our staff nutritionists work with you and your physician to promote good health with an understanding and appreciation of different cultures. Your optimum health is our goal.

Pediatric Care: Healthy beginnings are very important, and your children are our very special patients. As you guide your child to adolescence, we look forward to partnering with you through healthy-child visits, immunizations, school, camp or sports checkups, “working papers” physicals and sick visits, etc. We know that you will have confidence in the ability of our physicians, nurses and staff to not only meet but exceed your expectations as together we provide them with excellent healthcare, wellness and healthy choices. Accessing quality pediatric care has never been easier because one of our five offices is most likely near where you work or live.

Pharmacies: Joseph P. Addabbo Family Health Center wants to make healthcare as easy, available and convenient for patients as possible. One way we can do this is by providing on-site pharmacies that carry a wide variety of over-the-counter and prescription medications for everyone. The pharmacy specializes in the 340-B program that provides low-cost medication for the uninsured and underinsured. Professional and knowledgeable pharmacists provide prompt, polite and friendly service.

Podiatry: Our podiatrists provide medical diagnosis and treatment of foot and ankle problems such as bunions, heel pain, spurs, hammertoes, ingrown toenails, corns and calluses among other foot and ankle conditions. A podiatrist also renders care for sprains, fractures, infections and injuries of the foot, ankle and heel.

Social Work/Social Services: As part of our overall healthcare services, Joseph P. Addabbo Family Health Center provides social workers/case managers to help patients and their families identify resources to address barriers to care such as coordination of transportation, home care services, socioeconomic benefits, domestic violence, child welfare and veterans’ issues, etc.

Special Needs/Disabilities: If you or a loved one is living with special needs or disabilities, Joseph P. Addabbo Family Health Center offers trained care professionals who specialize in administering and providing comprehensive case management support in a highly compassionate manner. We can provide care for patients with autism, spina bifida, ALS, cerebral palsy, spinal injuries, brain injuries and accidents.

Women’s Health Services- OB/GYN: A woman’s health needs change and evolve during her lifetime. At Joseph P. Addabbo Family Health Center, we understand the importance of providing you with the most up-to-date medical care and expertise to keep you healthy throughout the decades. Our highly qualified OB/GYN physicians and nurses will help you understand the emotional and physical changes pregnancy can bring so you can have a positive experience. We also provide a wide range of services for women: FREE pregnancy testing, family planning (birth control), breast cancer screenings, STI and cervical cancer screenings, routine Pap smears plus pre- and post-menopausal counseling.

Women, Infants and Children (WIC) Program: The Special Supplemental Nutrition Program for Women, Infants, and Children is a federal assistance program of the Food and Nutrition Service of the United States Department of Agriculture for healthcare, nutrition education and advice for low-income pregnant women, breastfeeding women, and children under the age of five. Your first steps should be to have your Health Care Provider complete the WIC Program Medical Referral Form. The WIC Program of New York is administered by the New York State Department of Health.
