Joseph P. Addabbo Family Health Center patients have the right to access their health information according to state and federal laws. HIPAA regulations will cover the release of information to protect patient privacy.
The easiest way to obtain a copy of your medical records is to fill out the HIPAA-complaint request form and We’re Listening form Patient Authorization for the Release of Medical Information to a Third Party. You can complete the form online and click the submit button which will send the completed form directly to our medical records team. You can also complete the form online, print it and fax it to 718-318-9605, Attn: Health Information Management Department. If you are unable to fax the form, you can mail it to us or bring it to us at:

How to request your medical records
- When filling out the request form, please be aware of Section (9), which requires the date of service needed and which information is requested, for example: entire chart, labs, immunization records, etc.
- Please also see the Sensitive Information area, located in Section (9), which requires your initials authorizing the release of your drug, mental health or HIV-STD medical history.
- When requesting records for a minor, please fill out all pertinent areas of the form, including boxes (12) and (13). Box (12), Parent’s Signature and box (13) should indicate if you are the mother or father of the patient.
- Please provide proof of your ID, for example: state identification, driver’s license, passport or any other state- or school-issued ID.

Do I have to pay for my medical records?
Please ask the HIM (Health Information Management) professionals about any fees regarding your request for medical records.
How long does it take to get my records?
Normal processing time is between two to three weeks depending on the size of your medical file and scope of your request.
Can I pick up a copy of my records?
Yes. Please call ahead to make sure they are ready for pickup.
