Health Information Management (Medical Records)

At Joseph P. Addabbo Family Health Center, we believe every patient should have easy, secure access to their health information. Your medical records are protected under state and federal laws, including HIPAA, and we follow strict guidelines to safeguard your privacy.

 

Requesting Your Medical Records

You can request your medical records in several convenient ways. To begin, please complete the HIPAA Authorization for Release of Medical Information form. A copy of your valid ID (front and back) is required for all requests.

If you are considering transferring your care, please let us know what influenced your decision. Indicate “Yes or No” on the HIPAA form. If “Yes” then complete the online survey monkey to help us improve the care and services, we provide. You may also complete the manual survey We’re Listening Survey and submit via email or fax (information below).

1. Submit Your Request Online

  • Complete the HIPAA authorization form online. HIPAA-compliant request form
  • Click Submit to send it directly to our Health Information Management (HIM) team.
  • Email a copy of your ID (front and back) MR@Addabbo.org.

Important Information When Completing Your Request

  • Section 9: Be sure to specify the dates of service and the type of information you need (e.g., entire chart, lab results, immunization records).
  • Sensitive Information: Section 9 also includes a section requiring your initials to authorize the release of sensitive information such as mental health, substance use, or HIV/STD history.
  • Requests for Minors:
    • Complete all relevant sections, including Boxes 12 and 13.
    • Box 12 requires the parent or legal guardian’s signature.
    • Box 13 should indicate whether you are the mother or father.
  • Proof of Identification:
    Acceptable forms of ID include a state ID, driver’s license, passport, or school‑issued ID.

  • Do I have to pay for my medical records? Please speak with a Health Information Management professional for details about any applicable fees.
  • How long does it take to receive my records? Processing typically takes 2–3 weeks, depending on the size of your file and the type of information requested.
  • Can I pick up my records in person? Please call ahead to confirm that your records are ready for pickup.
