COVID-19 VACCINATION APPOINTMENTS

Due to the overwhelming volume of requests for vaccinations, the Joseph P. Addabbo Family Health Center (JPA) will be adhering to the following vaccination policy:

PEDIATRIC PATIENTS: The Joseph P. Addabbo Family Health Center (JPA) will be administering the vaccine to JPA pediatric patients age 12 to 17. You can call us at 718-945-7150 to make an appointment or Request a Pediatric Appointment at www.addabbo.org.

ADULT PATIENTS: If you are an unvaccinated patient at JPA’s Site 6 (120 Richards Street, Brooklyn), and wish to receive your first COVID-19 vaccination, please call 718-945-7150 and make your appointment.You can also request an appointment at Site 6 at www.addabbo.org. All appointments for second doses of the vaccine will be assigned at the time of the initial vaccination.:

Due to the large volume of requests and very limited appointment slots, immediate COVID-19 vaccination appointments are not available at any of our other locations at this time. Please check theNYC Vaccine Finder (www.vaccinefinder.nyc.gov) for other convenient locations that offer appointments or walk-in vaccinations.

THIRD DOSE OR BOOSTER SHOTS: The CDC has now recommended the Pfizer-BioNTech booster vaccine for certain populations. Please note that JPA will be contacting its immuno-compromised qualifying patients directly to schedule an appointment for a booster shot.

JPA is not offering booster shots to any other population at this time. We will update our vaccine availability policy as we move forward.

Please check the NYC Vaccine Finder for other convenient locations that offer appointments or walk-in appointments for both vaccines and/or Pfizer-BioNTech booster shots.

Although it is currently being considered, the CDC has yet to officially recommended Booster shots for any other population. JPA is therefore not offering booster shots to any other people at this time. We will update this policy as soon as more information becomes available.

The vaccinations will be provided at no cost to you. However, it is requested that you bring your ID and insurance card with you to an appointment.
