Thank you for your interest in having P. Addabbo Family Health Center attend your event. We attend many health fairs and events throughout the year in the communities served by our Centers. Our goal is to provide healthcare resources, prevention and disease management resources and education on the importance of having a healthcare home. We also offer informative educational presentations to community groups, organizations, churches and schools.
Please fill out the form below and click “Submit” to make a request. All programs and presentations are granted on a first-come, first-served basis. Members of our Marketing/Outreach staff will review your request and notify you when it has been approved. We will do our best to accommodate all requests.
